Once you've completed the steps on the Before You Begin page to set up your account, you are ready to log in to ProviderSource using your OneHealthPort Subscriber ID and password.

Use the Health Plan Notification Form to notify select health plans you have updated your credentialing information. 

We will route the form to the designated health plans on your behalf. Please allow the health plans seven business days before checking status. If you would like to check the status of your application, please contact the health plan directly.
