Important: LifeWise Health Plan of Washington commercial (group) plans are served only through Availity. Our Individual Plans are accessible through OHP Single Sign-On (SSO).

Individual Plans

If your patient's ID card has one of the listed group numbers (see sample ID card below) and/or their health insurance card includes the phrase "Individual Plan," sign in to the Individual Plan website by clicking on the blue "LWWA Individual Plans Login"  button above. 

Secure website access: When you sign in, you might see the message, "You have successfully logged into the Evolent application, but we do not find the Provider."  This happens because Evolent uses Identifi for provider authorizations. Identifi needs 1 to 5 days to set up your secure access. During that timeframe, you can visit our website for information on checking eligibility, submitting prior authorization, claims, and appeals for your individual plan patients. 

This sample ID card indicates the member is on an individual plan.
