Starting November 18, Premera will use Availity as its primary secure provider portal for checking eligibility and benefits, submitting claims, and getting claims status. You'll still need to use Premera's secure provider site for prior authorization. You can continue to use your existing OneHealthPort login after you register with Availity. **Premera - All Other Plans
This is our existing Premera website which remains available during the Availity transition. If you're looking for Individual Plan information prior to 2020, or if the patient's card doesn't say Individual Plan, log in to the Premera - All Other Plans website or log in to Availity.
***Premera - Individual Plans
If your patient's ID card has one of the listed group numbers (see sample ID card) and/or their health insurance card includes the phrase "Individual Plan," log in to the Individual Plan website.
If you've received the message, "You have successfully logged into the Evolent application, but we do not find the Provider," read these instructions to provide key services to your individual plan patients.
Note: Google Chrome is the best browser to use for the individual plan website.
